Please forgive my omission of the availability in obtaining the poinsettia tree donor form online through FHCCA (Fountain Hills Cultural and Civic Association) website.
The website is www.fhcultural-civic.org.
The form is also available at Gridleys of Fountain Hills and Sami Fine Jewelry & Unique Gifts, both located on The Avenue of the Fountains, and Kay Kinder’s Fountain Fashions at the Bashas’ shopping center on La Montana and Palisades Blvd.

The tree is scheduled to go up Thursday, Dec. 4, after 1 p.m. Come join us.
This year’s poinsettia tree jumpstarts the Christmas and Hanukah season on the newly-renovated Avenue of the Fountains Plaza. The location is great for picture taking.
Your donor card will be displayed on the A-frame near the poinsettia tree. Be sure to make your $20 check to the FHCCA and mail to Fountain Hills Cultural and Civic Association, P.O. Box 18254, Fountain Hills, AZ 85269.
