FH Family Practice joins 4C Medical Group

Fountain Hills Family Practice (FHFP), founded by Dr. Matthew Hummel, has integrated with 4C Medical Group, a physician-led medical group that provides integrated primary care services in Scottsdale, Carefree and North Phoenix.

Varesh Chaurasia, CEO of 4C Medical Group, said the practice has 38 providers and more than 50 staff members to make it one of the largest primary care groups in the Valley.

He said it is the only medical group to provide care in outpatient primary care, inpatient hospital care and skilled nursing facility settings.

“This offers our patients a widespread and diverse network of physicians and a better continuity of care across the healthcare delivery spectrum,” said Chaurasia.

The medical group will operate as Fountain Hills Family Practice in its current location in the medical campus, 16838 Palisades Blvd., Building C, suite 153.

Hummel has affiliated with MDVIP, Inc. to transition his practice to a concierge model of care in June.

“Concierge medicine” is becoming a rapidly popular form of health care. It allows unlimited visits, urgent care and an increased level of access to a doctor.

Hummel will remain in the same location next to 4C Medical Group.

He was one of the principal investors behind the construction of the medical campus that opened in 2008.

Dr. Joseph Summers, and physician assistants Erica Casselman, Russel Dodson and Haylen Trotta, will continue to provide primary care services at Fountain Hills Family Practice.

In addition, Dr. Jodi Novak and Dr. Rajesh Soni have joined the local team.

Soni is the outpatient medical director for 4C Medical Group, which now includes 18 primary care providers in four locations.

He graduated from Massachusetts Institute of Technology (MIT), earned his doctor of medicine degree at Mount Sinai Medical School in New York City and completed his residency at UCSF/San Francisco General Hospital in 1999.

In 2009, he and his wife moved from California to Fountain Hills with their three children. He will see patients at both the Scottsdale (96th St. and Shea Blvd. office) and Fountain Hills locations.

In addition, he will continue to provide care to skilled nursing facility patients at Fountain View Village where he has been caring for patients since 2012.

“We want patients to know that there will be no impact to continuity of care for the community, and patients can rest assured that they will have expanded access and more choices for their primary care needs,” said Soni.

Novak is originally from Crosby, Minn., and earned her doctorate at the University of Osteopathic Medicine and Health Sciences in Des Moines, Iowa, in 1999.

She completed her residency at Maricopa Medical Center in Phoenix and has practiced medicine in Arizona since then. She practices the osteopathic holistic approach to medicine, which integrates conventional and alternative therapies to prevent and treat disease and, most importantly, promote optimal health. She will practice full time at the Fountain Hills location.

Hummel opened his practice in 2002 as a one-man office, fulfilling a childhood dream to become a physician. The Chamber of Commerce selected him 2008 “Business Person of the Year.”

MDVIP, Inc., headquartered in Boca Raton, Fl., and operates a network of physicians who practice preventive medicine and personalized primary-care medicine. Patient-physician ratio is lower than the average in a traditional primary care practice.
